Mr. Corddry has worked in finance and accounting for more than 20 years. Prior to co-founding Lake Behavioral Health, Mr. Corddry was the CFO of a private hospital company for 12 years, helping to manage acute care and behavioral health services. In this capacity, he guided all financial planning, reporting, support services and business development activities. This included mergers and acquisitions, reimbursement, service line expansion, new facility planning, certificate of need activities, business office operations and service to support overall health care activities. Mr. Corddry has earned degrees from Michigan State University, Rollins College and Florida Southern College. Mr. Corddry is a Certified Public Accountant licensed in the state of Florida.
Ms. Hughes has over 25 years' experience in behavioral health, healthcare compliance, regulatory standards, clinical management, and leadership. She has held numerous positions in both the inpatient and outpatient arena addressing the needs of all age groups with an emphasis on geriatric care. She has served as Compliance and HIPPA officer for Behavioral Health and Substance Abuse services. She has administrative experience in private practice, community mental health, and hospital based behavioral health services. Ms. Hughes holds a Master of Science in Social Work from the University of Louisville and a Bachelor of Arts in Psychology from Western Kentucky University. She holds a license in Clinical Social Work for the state of Kentucky.
Ms. Robinson has worked in mental health settings, special needs programs, nursing facilities, and substance use services for over 20 years. Her experience includes practice management for mental health and substance use programs, executive director at residential facilities and day programs for those with developmental and intellectual disabilities, youth advocacy, social services director for long term care facilities, and home health services. In addition, she has expertise in Case Management and Human Resources. Ms. Robinson holds a Master of Science (MS) in Human Resources and Master of Science (MS) in Management from Sullivan University graduating Cum Laude with both degrees. She also obtained her Bachelor of Arts (BA) degree in Psychology from the University of Louisville and attended Murray State University, majoring in Psychology and minoring in Social Work.
Steven R. Shelton M.D. brings more than 30 years of psychiatric, behavioral health, and treatment of addiction experience. His experience serving as medical director for outpatient geriatric psychiatric programs, inpatient care, and corrections. Dr. Shelton graduated salutatorian from Western Kentucky University in Bowling Green, KY. Subsequently, he obtained a medical degree from the University of Louisville in 1985 and then completed an internal medicine and psychiatry residency there. In addition to currently holding certifications in adult psychiatry and addiction psychiatry from the American Board of Psychiatry and Neurology, Dr. Shelton holds a buprenorphine waiver for opioid treatment programs from the Substance Abuse and Mental Health Services Administration. With a diverse background in behavioral health, Dr. Shelton’s primary focus of interest is outpatient psychiatric care — including alcohol and drug abuse. Dr. Shelton is licensed to practice medicine by the Indiana Professional Licensing Agency and Kentucky Board of Medical Licensure.
Anna Reyes-Crawhorn, MSN, APRN, PMHNP brings over 23 years of quality mental health experience. She provides empathetic, compassionate, and nonjudgmental care to adults, children, and adolescents. Throughout her career, she has worked in a variety of settings, including the emergency room, directed several home health agencies, nursing practicum instructor at the University of Texas, and as a geriatric and child and adolescent psychiatric nurse in behavioral health hospital settings. She specializes in the treatment of mental health issues throughout the life span. Anna attended the University of Texas at El Paso and received her Bachelor of Science in Nursing. She graduated Sigma Theta Tau with honors and completed her Master of Science in Nursing from Walden University. Anna is board-certified by the American Nurses Credentialing Center as a Psychiatric-Mental Health Nurse Practitioner. Anna is licensed to practice in Kentucky and Indiana.
William Lavely, MSN, APRN, PMHNP has provided care in a variety of roles such as neurology, epilepsy, oncology, intensive care, orthopedics, and post-surgical care. In addition to serving as a RN instructor he has spent the last 6 years in treating mental health and substance use disorders. He specializes in the treatment of opioid use disorders, anxiety, and mood disorders, dually diagnosed with both mental health and substance issues, and is licensed in utilizing medication assisted treatment for substance use disorders. William began is education at 15 years old at the University of Louisville and received a Bachelor of Arts in Psychology/Sociology and a Bachelor of Science in Nursing. He later received his Master’s in Nursing at the University of Louisville with Post Master’s certification in Health Professions and Psychiatric Mental Health Nursing. William is licensed to practice in Kentucky and Indiana.
Rachel Roberts, MSN, PMHNP-BC began as a nurses assistant when she was a teenager to now serving patients across the lifespan. She specializes in seniors, mothers of young children, and those struggling with personality disorders. She understands not every patient needs medication and she has a laid-back approach to treatment. She utilizes evidenced based care for treating a full range of mental health and substance use concerns. Rachel obtained her Bachelor of Science in Nursing from University of Louisville, Master of Science in Nursing from Eastern State University with a Post Master’s certification in Psychiatric Mental Health Nursing. Rachel is licensed to practice in Kentucky.
John Palmer, APRN, PMHNP began his career as a nurses aid in long term care to obtaining his Masters in Nursing in 2019. He has provided behavioral health care for both inpatient and outpatient services. He specializes in the following areas: severe/chronic mental illness, mood and anxiety disorders, substance use disorders, trauma, ADHD, child and adolescent mental health, issues with aging, and general psychiatry. He earned his Bachelor and Masters of Science in Nursing at Indiana Wesleyan University John is licensed to practice Kentucky and Tennessee.
Meredith Kendell-Dunlow, MSN APRN, PMHNP Meredith loves providing patient centered care in both the inpatient and outpatient settings. She helps the patient identify goals and assists in working towards those goals. She provides passionate care to people through the ages. She began her mental health career as a Master Certified Hypnotherapist where she developed and taught at a hypnosis school in Idaho. Meredith has worked in the outpatient clinic setting serving ages 5 and up and as manager of an inpatient behavioral health hospital caring for ages 18 and up. As a nurse she has worked in various care units in Idaho, Washington, Texas, and Indiana, including oncology, orthopedics, ER Chest Pain/Stroke Coordinator, House Supervisor, Director of Medical/Telemetry/Orthopedics and Director of Education. She was always drawn to her love of behavioral health and validating those patients. Meredith has a Masters Degree in Nursing Leadership from Western Governors University and a Masters in Nursing from Northern Kentucky University and is board-certified by the American Nurses Credentialing Center as a Psychiatric-Mental Health Nurse Practitioner. Meredith is licensed in Indiana and Kentucky.
Lee Ann Keller, MSN, PMHNP-BC has served as an RN in a leading Emergency Department in Madisonville KY. In addition, her psychiatric and primary care career has included outpatient care in rural health care clinics and corrections. She specializes in holistic approaches, psychotropic medication management, the treatment of psychotic, mood, and anxiety disorders, as well as attention deficit disorder. She accepts patients at any stage of their lifespan. Lee Ann received her Bachelor of Science in Nursing from Murray State University, Master of Science in Nursing fromUniversity of Southern Indiana with a Post Master’s certification in Psychiatric Mental Health Nursing. Brittany Jackson PMHNP is a board-certified psychiatric nurse practitioner. Brittany obtained her Bachelor’s Degree in Psychology from Western Kentucky University in 2012, and is a 2020 graduate of Vanderbilt University Master’s of Nursing program. Brittany has devoted more than a decade of her professional career to helping individuals care for their mental health in a variety of settings including Rivendell Behavioral Health Services, LifeSkills, and Kentucky Psychiatry. Brittany’s training, as well as personal experiences, have prepared her to managing a variety of mental health conditions.
Joy Knopfmeier, MSSW, LCSW experience has included working with runaway youth, child protection, inpatient psychiatric treatment with children and adults, and outpatient practice working with all age groups. She specializes in the following areas: mood and anxiety; severe/chronic mental health issues; marriage/relationship issues; and adjustment issues related to life changes. Her management experience includes serving as the Clinical Administrator of a free-standing psychiatric hospital, and Clinical Director of a private psychiatric practice. In addition, she served on a professional task force that developed clinical programming for private psychiatric hospitals located throughout the country. She has served as an LCSW supervisor, and mentor for social work students. Joy received her Bachelor's Degree from the University of Evansville, and her Master's Degree in Social work from Kent School of Social Work at the University of Louisville. Joy is licensed to practice in Kentucky and Indiana.
Tracie Anderson, MSSW, LCSW has served as Assistant Clinical Director of Jefferson Psychiatric as well as providing therapy for Foster Care programs. Tracie earned her Bachelor’s degree from Central Michigan University and her Master’s degree from Denver University. She specializes in the treatment of chronic and severe mental illness, mood and anxiety disorders, gender identity adjustments, substance abuse, and trauma. Tracie serves all age groups as well as long term care residents. Tracie is licensed to practice in Kentucky and Indiana.
Larry Mudd, MSSW, LCSW has provided out-patient therapy services for private practice clinics for the past 23 years. He specializes in treating depression, anxiety, and bipolar disorder and working with couples and relationship issues. Larry earned his Bachelor’s degree in Sociology at the University of Indiana and his Master’s in Social Work from Kent School at the University of Louisville. He is a member of the National Association of Social Workers. Larry is licensed to practice in Kentucky and Indiana.
Aaron Szymanski, PMHNP-BD is a board certified Psychiatric Nurse Practitioner from Cookeville, Tennessee. He obtained his Bachelors of Science in Nursing at Tennessee Technological University in 2021, and later obtained his Master of Scient in Nursing at Tennessee Technological University in 2023. His background primarily consists of mood disorders, schizophrenia, spectrum disorders, and neurocognitive disorders. He provides services to all ages. Aaron is licensed to practice in Kentucky and Tennessee via Telehealth.
Cheri Hudson, MSSW, LCSW has worked with clients with a wide range of concerns including depression, anxiety, chronic mental illness, with a specialty in issues with aging. She has served in a variety of roles within Community Mental Health, Private Practice, Partial Hospitalization and Intensive Out Patient care for Geriatrics She obtained her Bachelors Degree in Social Work through the University of KY and later obtained her Masters Degree in Social Work at University of Lousville Kentucky University. Cheri is licensed to practice in Kentucky.
Brionna Pigram-MSW, CSW specializes in play therapy for children, with a wealth of experience in addressing grief, anxiety, depression, and ADHD. Her dedication extends to helping children with foster care and adoption, providing much-needed support and understanding. In addition, to her work with children, Brionna also has experience working with teens and adults navigating life transitions, demonstrating her versatility as a therapist. Brionna obtained her Bachelors in Psychology and her Masters in Social Work from Western Kentucky University. She is certified to provide services for Kentucky residents both in person and via telehealth.
Megan Kyle, LPCC has provided care in a variety of settings including community mental health, trauma centers, and other non-profit organizations. She specializes in adolescent and adult care addressing trauma, mood disorders, and anxiety . Her approach is using inclusive and collaborative care with an open minded, casual, and inclusive style. In addition, Megan provide Equine Assisted Therapy services. Megan received a Bachelor’s degree in Psychology and a Master’s degree in Clinical Mental Health Counseling from Western KY University. Shawn White, LPCC is a highly qualified healthcare services professional with autonomous licensure in the state of Kentucky. Shawn's licensure is as a Licensed Professional Clinical Counselor-Supervisor, and he specializes in dual diagnosis and substance use disorders. Shawn has significant experience in adult general mental health and providing clinical supervision. His previous roles have been Clinical Director, Clinical Supervisor, Therapist, and Targeted Case Manager in the Bowling Green, KY area. With a specialized focus on adults aged 20 and older, particularly middle aged-individuals. Crystal possesses a deep understanding of mental health challenges across different life stages. He graduated with his Bachelor’s Degree in Sociology and Criminology from Western KY University where is currently working on his Master’s in Social Work.
Elizabeth Boehme, BSW, has been working in case management for over three years and is currently pursuing her Master’s in Social Work an the University of Kentucky. She has training in trauma, law enforcement, and child protective services. She holds an Bachelor’s from the University of Pittsburgh and serves Kentucky residents eligible for Targeted Case Management.
Jimmy Smith is a board-certified psychiatric nurse practitioner. Jimmy obtained his bachelor's degree in Nursing from Middle Tennessee State University in 1982, and is a graduate of Regis College Master’s or Nursing program. Jimmy has worked as a Nurse Manager, Psychiatric inpatient, medical rehabilitation, and emergency medicine. He has 22 years of experience working with behavioral health. His specialty focus is pediatrics and substance use disorders. Jimmy serves as KY patients in person and virtually as well as IN patients virtually.
Stephanie Johnson-LPCA-has worked in both Community Mental Health and the private sector. She holds a bachelor’s degree in social work and a master’s degree in counseling. She is currently working on her dissertation for her Ph.D., which explores the stigma surrounding men’s mental health. She employs a combination of Cognitive Behavioral Therapy and Gestalt based on her client’s needs promoting holistic health and mental well being. In addition, she is a
certified yoga instructor for PTSD and children. She provides telehealth services of KY residents.
